Transitions Volunteers

Transitions needs volunteersTransitions is a free non-clinical support program for those with a prognosis of a year or so. Dealing with advancing illness is difficult, but the Transitions team can help ease worry, stress and fear. The professional staff assesses and case-manages each client, but it is the volunteer who is central to the client’s well-being and state of mind.


Volunteer opportunities include:

  • Companionship
  • Transportation
  • Help with errands
  • Light food preparation
  • Respite for the primary caregiver
  • Bi-weekly check-in phone calls
